Begin the Process
In order to streamline our Intake process and allow for a better, more secure, exchange of information, we have added Client Portal accounts to our services. Read the instructions below to begin the process.
If you have called our office, have spoken to our staff about an appointment, and are ready to start the Intake process, please begin by filling out the Google Forms found by clicking each of the buttons below.
If you have any difficulty accessing the forms below, use a different browser.
Patient Information Form—fill out and submit
Covid Policy—Read and e-sign (this is particularly important to know when you come into the office for an in-person visit as it outlines the very specific procedures to follow.)
Intake Information Form (the form you filled out previously above)—confirm information and e-sign
Insurance Card FRONT—upload a picture
Insurance Card BACK—upload a picture
PLEASE FINISH THIS PROCESS EITHER BEFORE YOU SCHEDULE YOUR INITIAL APPOINTMENT OR IMMEDIATELY FOLLOWING SCHEDULING THE APPOINTMENT. THAT WAY, IF YOU HAVE DIFFICULTY, WE CAN HELP YOU IN TIME SO THE APPOINTMENT WILL NOT HAVE TO BE RESCHEDULED!
The portal account will also have your current scheduled appointments available for you to view. You can access this information 24-hours a day at https://www.therapyportal.com/p/christian21045/.
If you are a current client and you do not yet have access to our client portal system, please contact our office staff at 410-995-5587 and ask them to send a portal invite to the appropriate email. You will then receive an email from TherapyPortal.com which will prompt you in how to set up an account.
Once you have done that, you will have 24-hour-a-day access to your current appointment schedule and access to a secure way to exchange important documents.
